• Graduate

  Most of the necessary paperwork as a graduate student at Illinois State University can be found on the Graduate School website. This site includes forms and information on degree audits, transfer credit, thesis or dissertation, scholarships, fellowships, tuition waivers, Graduate Assistantships, and grants.

  The list provided below are forms for the Mathematics Department that you may need throughout the completion of your degree.

  • Registrars Office MAT 400 Independent Study Form (File the Culminating Experience Form too)
  • MAT 400/490/498 Culminating Experience Form
  • Graduate Assistant Course Load Waiver Form (GA's who want to take less than 9 hours during last semester)
  • ISU Student Insurance Cancellation Waiver
  • Graduate Assistant Student Health Insurance Certification Form (every GA must file one of these each semester of an assistantship)
